Job Opportunity: Furnish Keyworker


‘Identify suitable interventions and liaise with support partners to develop bespoke individual action plans for customers who have identifiable multiple and complex needs. To inspire, motivate and empower them to establish and maintain effective and healthy lifestyles that lead to the reduced risk of homelessness. To support the Project Manager in achieving the contract KPI’s.’


Working with Individuals

Engage with and motivate individuals to identify and understand their needs. Work together to agree their priorities and design an action plan that is achievable and empowering, encouraging them to take the lead. Navigate them through their journey and explore with them the support options and choices available to them.

Work with individuals to enable them to acknowledge and meet their own needs, develop life skills and reduce vulnerability and risk of homelessness.

Agree and facilitate support programmes with individuals. Lead focused activities and services that that empower and enable. Provide advice and support to individuals in accessing existing services, clubs and self-help groups as a coordinated element of their action plan.

Provide up to date, accurate and relevant information with advice and guidance to increase knowledge and commitment to behaviour change linked to their identified

needs, with a focus on the core areas of money management, employment, healthy living and recycling skills.

Undertake continuous assessment of individual’s needs, barriers and issues together with overall caseload analysis and recommendations and present in report format to project manager on a monthly basis.

Manage a caseload of individuals, effectively supporting them to achieve targeted outcomes typically over a 6 month period
